Permission for Use of My Name and Digital Images of My Quilts On Website

Art Quilt Network (MVAQN) permission to use (1) my name, as a member of the MVAQN, and (2) digital images of my quilts on www.mv-artquilt.org, the website of MVAQN.

I understand that the digital images of the quilts on the MVAQN website may include any or all of the following:
1. the artist’s name
2. the title of the work
3. the completion date of the work
4. dimensions of the work
5. notes crediting the inspiration or techniques used in the work

I release the MVAQN from responsibility for any errors regarding my work or any misrepresentation of my work on the MVAQN website.

I will receive no remuneration of any kind for the use of the digital images of my quilts.

I may withdraw this permission at any time by notifying the leadership in writing.
